Please follow these steps to apply for a Workers Permit: 

• Download form AT-17. Complete only Part 1 with a parent or guardian signature.

• You will need a current physical dated within 12 months of the application date or the competed physicians form AT-16.

• Email both documents to EHS_Counseling@eastiron.monroe.edu or call the Eastridge High School Counseling Center at 339-1456 to set up an appointment to receive your permit.
