Notice of Availability - Form 1095-C
In compliance with the Affordable Care Act (ACA), East Irondequoit Central School District is required to furnish Form 1095-C, which outlines your health insurance coverage for the previous year.
Under the Paperwork Burden Reduction Act, Form 1095-C is now available upon request.
If you would like to receive a copy of your Form 1095-C, you may do so by completing the secure request form below.

Requests can be made at any time, and the form will be provided to you by March 3rd, or within 30 days of your request, whichever is later.
If you are an active employee with access to WincapWeb, you may login to download your 1095-C form at any time without submitting a request (most recent year made available by March 3rd).
If the information provided on your request is complete but does not match what we have on file, further verification may be necessary to confirm your identity.
Incomplete or missing information on form requests may not be accepted, received, or fulfilled to protect your privacy.
Your 1095-C can only be mailed or picked up from Central Office (600 Pardee Road, Rochester, NY 14609). You will be contacted for elected pickup option.
