Notice of Availability - Form 1095-C

In compliance with the Affordable Care Act (ACA), East Irondequoit Central School District is required to furnish Form 1095-C, which outlines your health insurance coverage for the previous year.

Under the Paperwork Burden Reduction Act, Form 1095-C is now available upon request.

 If you would like to receive a copy of your Form 1095-C, you may do so by completing the secure request form below.

  • Requests can be made at any time, and the form will be provided to you by March 3rd, or within 30 days of your request, whichever is later.

  • If you are an active employee with access to WincapWeb, you may login to download your 1095-C form at any time without submitting a request (most recent year made available by March 3rd).

  • If the information provided on your request is complete but does not match what we have on file, further verification may be necessary to confirm your identity.

  • Incomplete or missing information on form requests may not be accepted, received, or fulfilled to protect your privacy.

  • Your 1095-C can only be mailed or picked up from Central Office (600 Pardee Road, Rochester, NY 14609). You will be contacted for elected pickup option.
